Staff was advised and fully aware that our love one had a risk of falling, yet she fell out of her wheelchair on day 1. It was obvious from each FaceTime that she was declining rapidly and was getting very little interaction with staff and no physical exercise or physical therapy, which was ordered. When a follow up call was placed with the nursing supervisor they could not account for the injury.
